Job Description
The HEDIS Care Coordinator functions as an integral part of the HEDIS team and is responsible for partnering with external payors to assist with the closure of preventative and chronic disease measures for quality improvement and to promote better health outcomes. The coordinator works closely with the HEDIS NCM to schedule visits and to coordinate services for members with gaps in care all while providing close collaboration with the healthcare team. The coordinator plays a key role in improving Star measures and increased HEDIS scores by ensuring accuracy of all clinical data and reporting. Responsibilities for this role also include medical record abstraction and submission to the external payor, updating Health Maintenance and providing communication of progress to the HEDIS and leadership team. Minimum Requirements
  • High School diploma or equivalent required.
  • At least one year of experience in clerical or administrative capacity of health care or customer service setting.
  • Must possess computer keyboarding skills of at least 30 wpm and basic knowledge of Microsoft computer programs.
  • Excellent interpersonal and communication skills
  • Must be able to read and speak the English language
  • Considerable ability to provide superior customer service.
