The Pulmonary Rehabilitation Coordinator serves as the liaison between the patient and the referring physician and pulmonary rehabilitation team, managing and coordinating all aspects of the patient's care ensuring seamless coordination. The Pulmonary Rehabilitation Coordinator researches the patient's medical history and documentation, coordinates the pulmonary rehabilitation evaluation and subsequent sessions, and reviews the patient's insurance. Working closely with the pulmonary rehabilitation team, the Coordinator reviews patient information to confirm completeness and accuracy ensuring that all required clinical information (i.e. PFT's) is available prior to scheduling of evaluation. In addition, the Pulmonary Rehabilitation Coordinator ensures that all referrals, pre-authorizations, and pre-certifications, are received prior to services being rendered.

Responsibilities

  • 1. Serves as a contact between patients, families, referring providers, specialty teams, and community to ensure timely and appropriate referral processing confirming seamless coordination of patient care.
    • 1.1 Functions as a patient resource and advocate for the pulmonary rehabilitation program.
  • 2. Coordinates and supports tasks related to patient scheduling ensuring a prompt and courteous service to our patients, care givers, providers, and office staff.
    • 2.1 Provides specialized information related to the pulmonary rehabilitation program. Answers questions related to the program, criteria to enter program, and logistics regarding sites and hours of operation.
  • 3. Collaboration/Teamwork
    • 3.1 Delegate's responsibilities to appropriate members of the care delivery team based upon an understanding of others? roles, scope of practice, knowledge, and capabilities to ensure optimal utilization of personnel.
  • 4. Practice Development / Positive Impact
    • 4.1 Contributes to activities undertaken to meet patient care, department, and/or divisional goals and objectives.
  • 5. Leadership
    • 5.1 Communicates in a calm professional manner with patients, families, colleagues, members of the care team, and other hospital personnel, accepts and elicits feedback, and works toward resolving problems and clarifying issues and concerns.

Qualifications

EDUCATION

Associate's degree in Health Care Management, Public Health or Business Management required. Bachelor degree preferred.

EXPERIENCE

Two to three years of experience working in a health care related setting. Knowledge of EMR preferred..

SPECIAL SKILLS

Experience with computer systems required, including web based applications and Microsoft Office applications which includes Outlook, Word, Excel, and PowerPoint. Excellent communication (written and oral) skills. Ability to work collaboratively in a team to improve the operations of the program. Ability to provide a high level of customer service to patients, visitors, staff, providers, and community providers in a professional, service-oriented, respectful manner using skills in active listening and problem solving. Ability to work in a high volume work environment and multitask.
