The Clinical Social Worker-LCSW job assesses and triages patient and family needs. Processes referrals and schedules clinic appointments. This role assists patients and families with feeling comfortable throughout clinic appointment. This role assists with interviewing patient and family members. This role ensures mental health resources are available to all patients and family members. This role ensures families follow up with recommended services. This role also assists patient and family with health insurance issues and difficulties obtaining prescribed medications.

The Clinical Social Worker-LCSW makes appropriate referrals. Encourages use of trauma-informed practices. This position ensues law enforcement and Child Protective Services (CPS) understand medical terminology and recommendations. This position also acts as a liaison between CPT, law enforcement and CPS to ensure ongoing collaboration.

The Clinical Social Worker acts as a liaison between medical team members. This job assists with developing best practices and protocols. This job also maintains medical and legal documentation. Licensure, Certification, or Registration Requirements for Hire:

Experience REQUIRED:
Minimum of five (5) years of social work experience.
Experience PREFERRED:
Previous work experience providing case management and discharge planning in an acute care setting or outpatient setting.
Previous work experience providing individual, group, and/or family therapy in an acute care setting or outpatient facility.
One (1) year in a lead role.
Education/training REQUIRED:
Masters of Social Work Degree from a Council on Social Work Education (CSWE) accredited program.
Education/training PREFERRED: N/A
Independent action(s) required:
Conducting clinical assessments to include mental health symptoms, substance use issues, housing concerns, financial and employment problems, abuse and domestic violence history, religious faith or spirituality, military status, medical issues, and any other, social, or health issues.
Providing proactive development, implementation, and ongoing revision of an interdisciplinary plan of care for patients, specifically addressing the patients' social issues and psychiatric reasons for admission such as suicidality, homicidally, psychotic disorder symptoms, and mood disorder symptoms.
Providing individual, group, and family therapy, in order to appropriately assess, diagnose, and treat patients affected by social issues and mental health disorders.
Implementing appropriate therapeutic interventions to assess, diagnose, treat, and provide resources for mental illness, substance use, housing concerns, trauma, domestic violence, sexual assault, abuse/neglect, guardianship and other complex psychiatric, social, or medical issues.
Collaborating daily with patients, their families, the interdisciplinary team, and community agencies to facilitate a safe discharge plan.

The clinical social worker for Home Based Primary Care (HBPC) is part of an interdisciplinary team that provides primary care to home bound adults in the community within a 15 mile radius of the VCU Health medical campus. This team member will be expected to make home visits to patients and provide clinical case management, advance care planning, and behavioral health. The social worker is integral to the interdisciplinary team providing support to the team and working with patients to improve their complex psychosocial situations to reduce risk for readmission to the emergency room or inpatient unit.

It is expected that this team member see patients in person, Mon-Fri during normal business hours, with no on-call or weekend hours required. Business casual attire is expected. The Health System follows Centers for Disease Control and Prevention (CDC) guidelines for hand hygiene and fingernail care.
