Overview Social Worker LISW - Care at Home Quad Cities Monday-Friday 8a-5p .80 FTE Full Time Benefits, 32 hours/week Maintains the standards of social work excellence in accordance with the scope and practice and standards of care set forth by the proper state chapter, NASW Standards for Social Work Practice in a Health Care Setting and the Social Work Code of Ethics. Responsibilities Position Overivew: Conducts comprehensive psychosocial and behavioral assessments. Develops treatment plans and provides appropriate interventions and therapy for behavioral health illnesses. Assesses, diagnoses, and treats social and emotional factors related to patients' illnesses and care needs. Creates patient-centered care plans within the scope of Hospice. Offers information and referral services to individuals and agencies. Adopts a holistic approach, considering physical, psychosocial, spiritual, educational, and safety aspects. Actively participates in interdisciplinary team meetings and plan development. Qualifications Education: Master's degree in social work with accreditation by the council on Social Work Education Experience:One year clinical experience in health care Preferred: Previous hospice and home care experience License(s)/Certification(s): Current appropriate State License. Valid driver's license when driving any vehicle for work-related reasons. Mandatory Reporter: Child & Dependent Adult Abuse Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities. Licensed Independent Social Worker/Licensed Clinical Social Worker - licensure in good standing as required by applicable law. CPR Certification Knowledge/Skills/Abilities: Strong interpersonal skills and customer service skills. Strong verbal and written communication skills. Ability to take initiative and exercise independent judgment, decision making and problem solving expertise. Ability to work as a collaborative team member.
