FULL SALARY RANGE: $50,194 - $78,428/annually
LOCATION: Hybrid; telework and in-person at the Northern Service Center, West St. Paul

A Community Health Worker works with clients and families to help connect them to needed county and community resources, provides community education, works to increase access to primary care through culturally competent outreach, and makes referrals and coordinates services as needed for clients and families.

Minimum Qualifications

NEED TO HAVE QUALIFICATIONS

  • Associate degree (A.S., A.A.) or two year technical training beyond high school AND
  • Certification as a Community Health Worker in Minnesota or ability to obtain within first 6 months of employment
  • Valid Driver's License

NICE TO HAVE QUALIFICATIONS
  • Experience in community outreach and engagement
  • Bachelor's Degree in health-related field
  • Oral and written fluency in another language besides English


Duties & Responsibilities

These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position's management approved work schedule is required for all positions.

  1. Provide outreach, education, and resources to individuals and communities in home visits through the Family Home Visiting program or community settings.
  2. Document home visits and outreach activities in computer programs and/or the Electronic Health Record (EHR).
  3. Attend team meetings and provide input to team discussions and work plan development.
  4. Promote regular medical and dental checkups for C&TC eligible individuals and families in community settings.
  5. Provide advocacy for clients and families when barriers are encountered to health care access.
  6. Plan and present basic preventative health and safety information to children and adults in community settings.
  7. Engage clients in obtaining skills to navigate the health care system.
  8. Perform public health emergency preparedness functions as trained and assigned within the department's emergency response plan.
  9. Provides mentoring or precepting of new staff or students as assigned.
  10. Perform other duties as assigned.

Knowledge, Skills & Abilities and Work Environment

  • Knowledge of data privacy and HIPAA regulations and assigned public health program goals and principles.
  • Knowledge and ability to navigate available community resources.
  • Knowledge of the health care delivery system.
  • Working knowledge of public health principles, programs, and services.
  • Ability to communicate effectively verbally and in writing.
  • Ability to use technology needed to perform the functions of the job.
  • Ability to work in office and community with minimal supervision.
  • Ability to engage and present information and materials to individuals and small groups.
WORK ENVIRONMENT: Work is performed in a county office building and in the community throughout Dakota County. Travel and reliable transportation is required. May need to transport or move objects weighing up to 50 pounds on an occasional basis and up to 20 pounds frequently, exposure to common illnesses and hazardous environmental conditions and materials. Types of equipment used includes: telephone, printer/copier, personal computer, Home Safety kits, car and booster seats and audiovisual equipment.

SELECTION PROCESS: The examination/selection process for this classification will consist of a rating of your training and experience from the application materials submitted. The top scoring candidates will be forwarded to the hiring department for further consideration.
