JOB SUMMARY
This job will assist the patient, family and care giver to overcome healthcare barriers (including but not limited to social, financial, or physical barriers.) Responsible for conducting on-going assessments of need and facilitate referrals to additional Support Services throughout the Oncology continuum of care. Using the nursing process and evidence based knowledge, the Oncology Nurse Navigator provides education and resources to facilitate informed decision making (by the patient and/or family.)
Preferred oncology nursing experience and oncology certification. The navigator will support cancer patients in the Urology clinic which will bring cancer support to a population of cancer patients not physically treated in the cancer center. The navigator will work with the intake team for the Urology clinic to improve access by immediate chart review, guidance of referrals to the cancer center for metastatic patients and expediting patients that should be seen timelier. This navigator will work closely with the GU ONN in the cancer center for smooth transition of care through the patient's cancer journey. The navigator will assess for SDOH and make appropriate referrals to support services as needed.

EXPERIENCE AND EDUCATION
Required
- Education
Associate's Degree Graduate of an approved/accredited nursing program either by a governmental authority, nursing board, or by a licensing authority that has jurisdiction over approval/accreditation of nursing programs (Texas BON 217.1)
- Experience
3 years oncology nursing experience
- Licenses and Certifications
(RN) REGISTERED NURSE and
(BLS) BASIC LIFE SUPPORT course accredited by the American Heart Association (AHA) or American Red Cross (ARC), are required. and
Preferred
- Education
Bachelor's Degree Bachelor of Science in Nursing.

(OCN) ONCOLOGY CERTIFIED NURSE or
(CPHON) CERT PED HEMATLGY ONCOLGY NURS or
(CBCN) CERTIFIED BREAST CARE NURSE or
(BMTCN) Bone Marrow Transplant Cert RN
JOB DUTIES
- Assesses for any barrier to care through the cancer care continuum to address patient, care giver, or family needs.
- Serves as point of contact for provider, patient and care giver.
- Serves as patient advocate to assist the patient and care giver through the healthcare system.
- Facilitates education of patient and staff by developing appropriate education programs, resources and tools to address needs.
- Provides education, resources and referrals to internal and external resources based off of assessment need.
- Facilitates shared decision making, ensuring the patient, family and care giver are fully informed of treatment intent, treatment and outcomes.
- Promote care planning by encouraging patients to convey their wishes and goals of care. Works with patients to formulate questions to discuss with their healthcare provider.
- Facilitates clear and concise communication among providers, departments and patient.
- Coordinate multidisciplinary planning and facilitate care conferences, as necessary.
- Participates in outreach projects to help close patient care gaps.
- Performs other duties as assigned.
