Essential Job Duties |
This position will provide administrative support to two 1915 Home and Community-based Services waivers, known as the Community Alternatives Program for Children (
CAP/C) and Disabled Adults (
CAP/DA). The
CAP waivers are authorized under section1915 of the Social Security Act and complies with 42
CFR * 440.180. These programs provide a cost-effective alternative to institutionalization by supporting a physically disabled Medicaid beneficiary to remain in or return to a home and community-based setting.
The position will assist in identifying opportunities for all North Carolinians with severe and chronic physical disabilities to access home and community-based services and other medical and non-medical services using 1915
HCBS waivers. This position will also track individuals determined to be dually eligible to evaluate alignment of their Medicare and Medicaid services to ensure access to needed care. This position will use data to evaluate barriers of access to
care for
LTSS and dually eligible Medicaid beneficiaries. This position will also analyze immunization, preventive services, physician visits, well childhood visits, and hospital admissions follow-up data of participants in 1915
HCBS waivers to compare against similar
Medicaid beneficiaries to assist with the development of an annual action plan and goals for improving health equity in the 1915
HCBS programs. |

Other Work/Responsibilities |
- Develop an annual action plan and goals for improving equity in the waiver program
- Perform analysis on current equity performance using data mining to identify chronically ill/disabled children in underutilized areas across the state to identify a health equity plan for enrollment and management of participant wait time.
- Develop and circulate a waiver target population guide to educate referral agents and professionals, including social workers, physicians, discharge planners, staff of community-based organization, nurses, and clergy, on identifying candidates
who are appropriate for home and community-based services and waiver enrollment

Minimum Experience/Education |
- Master's degree in business administration, computer science, data science, management information systems, or closely related field from an appropriately accredited institution and three years of work experience in technology, report delivery, and BI strategy; and one year of long-term services and support and
home and community-based services; or
Bachelor's degree in business administration, computer science, data science, management information systems, or closely related field from an appropriately accredited institution and four years of work experience in technology, report delivery, and BI strategy; and one year of long-term services and support and home and community-based services; or
- Licensed to practice as a Nurse in the State of North Carolina and three years of long-term services and support and home and community-based services; or
- an equivalent combination of education and experience.

- Demonstrated ability to work with a team
- Considerable knowledge in working with health records
