We Are Inspired to Serve. Join us!The physical therapists is responsible for proving physical therapy services and for adherence to all conditions outlined in the personnel services agreement.

Responsibilities:

  • Assessing and evaluating therapeutic, rehabilitative, functional status and participation in the development of the total plan of care. Evaluates home environment.
  • Assess for muscle strength, mobility, gait, ROM - potential for rehab.
  • Directing physical therapy treatment.
  • Initiates physical therapy program and instructs other personnel and/or family /caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
  • May train patient in the use of prosthetic device.
  • Participates in discharge planning for patient and in peer consultation process.
  • Provides physical therapy consultation to home families/caregivers when indicated and provides Inservice education programs for nursing organization personnel as needed.
  • When therapy is only skilled service, instructs, supervises and evaluates home health aide care every two weeks.

Requirements:

  • Bachelor's Degree in Physical Therapy approved by an accredited organization
  • PT License and CPR certification, if required by State
  • 2+ years experience.

Compensation Pay Range:

$38.17 - $58.03

Reasonable Pay Estimate

A reasonable estimate of the pay range for this position is $38.17 - $58.03. There are numerous factors taken into consideration in determining the actual offered rate of pay, including but not limited to: job-related qualifications, experience, skills, education, geographic location, and consideration of internal and external equity.
