Location: West Allis, WI | Full-Time | Flexible Hours | Hybrid Work Option Available

Join our passionate team at PSG/CIP, a dynamic and community-focused social services organization providing innovative mental health programs across eastern Wisconsin!


Position Overview

We are seeking a compassionate and driven Care Coordinator to support individuals with significant mental health and substance use challenges. You'll deliver wraparound, strength-based services that empower clients in their homes and communities, using a recovery-oriented and person-centered approach.


Key Responsibilities


  • Conduct comprehensive strengths and needs assessments during intake and throughout services



  • Develop and implement individualized Recovery Plans of Care



  • Coordinate and lead team meetings with service providers and support networks



  • Provide crisis intervention and safety planning as needed



  • Attend court hearings, psychiatry appointments, and provide advocacy support



  • Safely transport clients to appointments when necessary



  • Build strong formal and informal support teams




Position Details


  • Typical Hours: Monday-Friday, 9:00 AM - 5:00 PM (some flexibility)



  • Travel: Local travel required; mileage reimbursement provided



  • Work Setting: Hybrid/remote scheduling available



  • Pay:
    * $23.00/hour - Bachelor's Degree
    * $26.00/hour - Master's Degree




What You'll Need


  • Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related field (required)



  • Valid driver's license, reliable daily transportation, and insurance



  • 1+ year experience working with individuals with mental health or substance use needs (preferred)
