Responsible for facilitating the intake process for new patients. The Intake Specialist serves as the central coordinator for referral management, insurance verification, and initial assessment scheduling. Conducting thorough patient assessments, they determine eligibility for Home Health and/or Hospice services and facilitate the transition from referral to admission. Through effective communication and collaboration with healthcare providers and patients, they ensure a seamless intake experience while maintaining compliance with regulatory standards and organizational protocols. By efficiently initiating and coordinating patient-centered Home Health and/or Hospice services, the Specialist plays a pivotal role in ensuring timely access to care and resources for individuals in need.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Associates Degree or Certification in Nursing 2. Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC). PREFERRED QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelor's degree in nursing. EXPERIENCE: 1. Two (2) years of Home Health and/or Hospice experience. 2. Two (2) years of experience in utilization review/chart audit. Manage the intake process for new patients, including receiving referrals from healthcare providers, insurance verification, and scheduling initial assessments. 2. Serve as the primary point of contact for patients, families, healthcare professionals, and referral sources regarding Home Health and/or Hospice services. 3. Ensure accurate and timely documentation of patient information, including medical history, assessments, care plans, and communication with other healthcare providers. 4. Collaborate with interdisciplinary team members to develop individualized care plans based on patients' needs, goals, and preferences. 5. Coordinate services and resources needed to support patients' care plans, such as medical equipment, supplies, therapy services, and community resources. 6. Provide education to patients and families about their condition, treatment options, self-care techniques, and available support services. 7. Monitor patients' progress and response to treatment, reassessing needs as necessary, and adjusting care plans accordingly. 8. Coordinate transitions between care settings, such as hospital discharge to Home Health and/or Hospice services, ensuring continuity of care and smooth transitions. 9. Participate in quality assurance activities, such as audits, reviews, and performance improvement initiatives, to ensure compliance with regulatory standards and quality outcomes. 10. Collaborate with healthcare professionals, including physicians, nurses, therapists, and social workers, to ensure coordinated and comprehensive care for patients.
