I240 Service Road, OKC OK 73135 Shift: PRN Responsibilities The OHH Social Woker/ Case Manager/Discharge Planner is responsible for all activities associated with the discharge planning and coordination of care and service of individual patients and/or patient populations. Promotes effective utilization of resources and assume a leadership role on the multidisciplinary team to achieve patient and physician satisfaction and optimal outcomes. Must be able to work with all disciplines to meet patient needs from admission through discharge. Will perform duties and responsibilities in a manner consisted with the mission and values of the Oklahoma Heart Hospital. Weekend flexible hours and additional hours optional during the week. Qualifications Education: Graduate of an accredited social work program. Experience: Minimum of one (1) years of hospital based social work experience. Experience in hospital case management, discharge planning or social work a plus. Willing to pursue certification in Case Management. Proficient in Computer Knowledge.
