Job Summary: The Project Lead-Post Acute Care Coordination leads efforts to optimize post-acute care utilization at the time of discharge care and across the outpatient continuum. This role focuses on de-escalating unnecessary post-acute care, ensuring appropriate placement of patients into lower levels of care when clinically indicated, and training interdisciplinary staff on effective patient placement strategies and payor alignment. Serves as a key liaison among outpatient care teams, payors, and post-acute providers to improve outcomes, reduce costs, and enhance patient experience. Additionally, this person is a key point of contact for training and resource education for inpatient therapists, physicians, social workers and case management. Education and Experience: Bachelor's degree in Nursing, Social Work, Healthcare Administration, or related field required, Master's degree preferred. Ten years of experience in case management, post-acute care, or care coordination required. Knowledge and Skills: Knowledge of CMS regulations, post-acute care criteria (e.g., InterQual, MCG), and managed care protocols. Strong presentation, communication, and staff education skills. Demonstrated history of increasing responsibility. Possesses excellent dexterity and analytic skills. Ability to plan and organize technical work, manage workload, operate technical instruments and equipment, input data and type. Possesses clear speaking abilities in English. Ability to use available resource material independently to resolve and document challenges constructively. Possesses excellent and effective verbal and written communication skills in English matching the style to the message and the audience. Maintains excellent interpersonal relationships with all customers (e.g., staff, patients, vendors). Provides staff and management with timely information, constructive and actionable feedback. Fosters cooperation and collaboration actively solicits opinions from others and builds respect and trust. Explains and teaches. Responds to guidance constructively. Detail oriented. Able to reason and solve problems effectively. Able to organize work, to service multiple requests and tasks at one time, while maintaining quality and a professional manner. Self-motivated and works well with minimal direction. Articulates logical and well-founded arguments that support conclusions. Licensures and Certifications: Current licensure in a clinical discipline (e.g., RN, PT, LCSW) required. Other clinical licenses may be considered based on relevant experience.
