Social Worker | Acute Care

Scheduled Weekly Hours:

Emplify Health by Gundersen Health System is looking for a full-time Social Worker to work across acute care units. Work with a variety of patient populations and grow your social work skills!

What you will work:

• Full-time, 80 hours every 2 weeks

• Monday - Friday, day shifts

• One holiday per year

What you will do:

• Assessment and treatment planning

• Discharge planning

• Provide patient and family education and support

• Document

What you will need:

• Bachelor'sdegree in Social Work or in a related field

• WI Social Worker license

• 1 year of previous experience in human services or health care
