Nurse Practitioner (Per Diem)

Field-Based position will cover Queens, NY and Brooklyn, NY.

Per Diem Rates:

On Call - regular days Monday - Friday (shift 5pm - 8am). Payment is $215

On Call -Weekends and Holidays. The shifts are from Saturday 8am -Monday 8am (or morning after the holiday). Shifts on weekends and holidays are 12 h (8am -8pm and 8pm -8am), payment is $240 /12h shift.

Weekend facility rounds or urgent cases overtime ( ISNP/IE-SNP ) -$ 90 /1hour ( usually 4 hours /facility )

Community Program MJHS Plus overtime -urgent Home Visit Initial /Chronic Care Comprehensive -$180, urgent home visit follows up visit -$150

Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPN's who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home.

In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complex
physical examination and psychosocial needs, self - management education & tools to better manage patient's multiple conditions. As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.
  • Masters Degree in Nursing, graduate of accredited Nurse Practitioner program required
  • One to two years clinical experience in role of nurse practitioner required
  • Minimum one year in geriatric specialty
  • Previous clinical RN experience in acute, subacute and/or long term care setting preferred
  • Prior experience in manage care a plus
  • NYS RN/NP License, Registration, DEA certificates
