SUMMARY: We are currently seeking a Case Management Manager to join our Transitional Care team. This is a full-time role.

Purpose of this position: Responsible for managing and coordinating the case management program, including clinical coordination and back up for utilization management activities to achieve departmental and organizational goals.

RESPONSIBILITIES:



  • Plans and manages case management services
  • Establishes standards for services provided and monitors compliance / quality of services
  • Manages personnel responsibilities including recruiting and selection of staff, training, and personnel evaluations. Develops, supports, and evaluates case management staff competencies related to clinical coordination to achieve optimal departmental and organizational outcomes.
  • Promotes effective relationships with multidisciplinary teams
  • Prepares plans related to and achieves annual departmental goals including clinical improvement, patient satisfaction, employee engagement, financial vitality and growth and prepares and maintains related reports as needed. Develops, maintains, and evaluates information/data collection systems related to case management. Collects data and generates reports related to process and outcome measures. Takes action based on this information to achieve goals
  • Schedules employees as necessary to meet patient needs. Evaluates and balances workload of clinical coordinator to optimize their effectiveness
  • Develops policies and procedures related to case management program
  • Maintains responsibility for department's operational excellence; ensures department delivers quality services in accordance with applicable policies, procedures, professional standards, and regulations
  • Manages the financial resources for case management services by monitoring operational expenses, establishing and maintaining cost control processes and responding to variances as needed
  • Develops and manages the department budget in conjunction with organizational goals and objectives
  • Promotes effective working relations both interdepartmentally to promote system-wide continuity of care
  • Assesses patient and provider satisfaction in area of responsibility and takes steps to ensure that satisfaction
  • Works collaboratively with other leaders to address complex patient discharge issues
  • In collaboration with other department leaders, maintains contacts with outside resources in the postacute care network to provide for continuum of care needs of patients

QUALIFICATIONS:

Minimum Qualifications:



  • Graduation from an approved school of nursing with Bachelor's degree in Nursing
  • At least five years of professional nursing experience
  • Master's degree in nursing or healthcare administration (or equivalent) or enrolled in similar program with significant progress towards degree or management experience
  • At least 2 years Case management experience
  • At least two years of management experience in health care or related field with experience leading and working as part of multidisciplinary teams


Preferred Qualifications:



  • Case Management certification
  • 5 years of management experience in leadership preferred

  • Ability to effectively manage company resources (i.e. budget and personnel)
  • Skilled in overseeing the efforts of high-level department individuals
  • Superb ability to motivate employees and inspire positive change within department
  • Innovative thinker; Ability to contribute new ideas that support organizational goals
  • Skilled in managerial duties (i.e. hiring, firing, performance appraisals, pay reviews)
  • Skilled in modeling company values through daily interactions within the department, particularly with regards to health and safety
  • Capable of supporting the work of the department
  • High standard of ethics, discipline, and professionalism
  • Skilled in fostering a positive workplace culture and building inclusive workplace teams


License/Certifications:



  • Possession of a valid license as a registered nurse issued by the State of Minnesota
