DESCRIPTION

Decypher is seeking a Clinical Pharmacist candidate to provide support to the Holloman AFB, NM.

QUALIFICATIONS:



  • Degree: Doctor of Pharmacy (Pharm.D.)
  • Education: Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE) or a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate and active licensure from a U.S. state, territory, or district.
  • Internship/Residency: Completion of a clinical pharmacy internship, residency and/or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy.
  • Licensure: Current, full, active, and unrestricted license to practice as a clinical pharmacist.
  • Requirements for additional Pharmacist privileges include one of the following:

    • Certification by the Board of Pharmacy Specialties in one or more recognized practice areas appropriate for the privileges requested (e.g., Pharmacotherapy, Ambulatory Care Pharmacy, et.); OR
    • Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health-System Pharmacists or American College of Clinical Pharmacy; OR
    • Completion of a Post-Graduate Master of Science degree with emphasis in medication therapy management or direct patient care.




RESPONSIBILITIES:



  • Evaluate patients' responses to medications and help plan appropriate drug therapies based on physician diagnoses.
  • Order and review lab tests to monitor the effectiveness and safety of medications.
  • Collaborate with medical staff to start, adjust, or stop medications for chronic conditions (e.g., hypertension, diabetes, asthma).
  • Manage medications that require regular monitoring and dosage adjustments (e.g., antithrombotics, phenytoin).
  • Administer prescription and over-the-counter medications per established protocols.
  • Assess nutritional needs and prescribe therapeutic nutrition (enteral/parenteral) for hospitalized patients.
  • Review medical and medication histories to identify and resolve drug-related issues.
  • Work closely with other healthcare professionals to coordinate patient care.
  • Perform comprehensive pharmacy services, including reviewing prescriptions for accuracy and safety.
  • Ensure proper dosing, check for drug interactions, and verify allergy information.
  • Oversee the preparation and dispensing of medications by pharmacy technicians.
  • Provide training to clinical staff on new medications, best practices, and treatment comparisons.
  • Consult with specialists to optimize pharmaceutical care.
  • Document significant drug interactions and communications with prescribers.
  • Report adverse drug reactions in line with accreditation standards.
