In this Monday- Friday 8am-4pm position, the Rehabilitation Assistant willl be responsible for assisting the Director of Rehabilitation with clerical tasks to support daily departmental and operational processes. Responsibilities Rehabilitation Assistant Essential Job Functions 1. Attends department huddles/meetings (in coordination with ADOR/DOR's schedule) 2. Coordinates and maintains therapy referral process a. Inputs referrals into spreadsheet and schedules for completion b. Manages referral process and ensures referrals are scheduled properly c. Logs/files referral completion outcomes and maintains long term care program audit list upon completion of referrals d. Compiles conversion rates weekly and submits to Director (Clinical Development Worksheet) 3. Conducts Casamba reconciliations a. Discharges residents, including input of resident demographic information from system b. Monitors ADT to maintain in-house versus discharged status of residents c. Sets payor for admission upon new admits as directed Job Title Rehabilitation Assistant Department Therapy FLSA / Grade / Job Code Non-Exempt 2 REHCOR Revision dates 12/2022 Reports to Director of Rehabilitation Job Summary The Rehabilitation Assistant is responsible for assisting the Director of Rehabilitation with clerical tasks to support daily departmental and operational processes. Educational Requirements and Qualifications * Experience with Microsoft Office applications including email; ability to learn company software systems * Associate degree preferred 4. Maintains therapist scheduling as directed a. Updates Casamba calendar for time off/per diem coverage schedule b. Maintains weekend coverage schedule c. Pre-assigns therapists to treatments in prep for next day scheduling d. Reconciles Casamba payroll report with Kronos 5. Processes insurance authorizations a. Completes authorization requests b. Assists in obtaining consent from families for therapy services c. Tracks visits, requests additional visits as needed d. Assists with appeals / denials 6. Monitors documentation compliance a. Runs various reports to include, but may not be limited to: incomplete documentation reports, functional limitation reports, incomplete outcomes reports; identifies therapist accordingly; notifies for completion b. Coordinates with Medical Records to follow-up on missing/incomplete documentation c. Reviews Care plan for accuracy d. Delivers paperwork to units and reviews signatures completed by physicians e. Notifies staff of D/C POC completion and monitors compliance f. Uploads patient documentation to PCC 7. Maintains quarterly assessment process as directed a. Assigns quarterly assessment in Casamba scheduler daily b. Audits completion of assessments 8. Coordinates DME process a. Obtains DME orders from physician/NP b. Coordinates purchases and arranges delivery of equipment and supplies c. Diabetic Shoes for entire facility - obtain scripts, provider notes, diabetic exams, contacts vendor and schedules visits to the facility d. TLSO, LSO and AFO's - obtain scripts, contact vendor, send insurance info and schedule appointment for orthotist to come to the facility e. Coordinates home evaluations as needed 9. Coordinates Nursing Part B supporting documentation as directed a. Updates Part B caseload/nursing supporting documentation needs and distributes to units 10. Addresses and completes PRN Tasks as directed by the DOR a. Initial preparation of ADR claims i. Compiles supporting documentation required for ADR claim ii. Pre-populates ADR letter based on denial information for DOR to complete summary of medical necessity b. Miscellaneous Tasks i. Acts as point-of-contact for rehab daily calls ii. Coordinates purchasing process iii. Assists with monitoring processional options account iv. Assists with maintaining staff and therapists compliance records v. Provides general IT assistance vi. Requests maintenance assistance via TELS 11. Coordinates care plan process with social services a. Ensures therapists availability for ODT care plan meetings b. Schedules appropriate training with family members and therapists 12. Provides oversight to designated non-clinical staff to include orientation and training 13. Assumes Outpatient Intake Coordinator responsibilities as needed 14. Coordinates Student Observations, if applicable 15. Utilizes electronic timekeeping system as directed 16. Arrives to work on time, regularly, and works as scheduled 17. Recognizes and follows the dress code of the facility including wearing name tag at all times. 18. Follows policy and procedure regarding all electronic devices, computers, tablets, etc. 19. Supports and abides by Elderwood's Mission, Vision, and Values 20.
