Job Title: Licensed Practical Nurse II Duration: 3+ Months to long term Location: Edison, NJ 08837 Schedule: 1^stShift (7:00 AM to 3:00 PM) Role: On Site Job Description: Support triage functions by responding to patient/family calls and communicating with field staff and providers. Prepare reports for quality assurance and performance improvement (QAPI) initiatives. Assist with audits, surveys, and internal reviews to ensure adherence to regulatory and organizational standards. Participate in the preparation of the interdisciplinary team (IDT) meetings, ensuring updated patient records and reports are available. Monitor and communicate any concerns related to clinical documentation, patient status, or care coordination. Act as a liaison between physicians, hospice staff, patients, and family members to facilitate effective communication and continuity of care. Coordinate communication related to medication orders,DME, supplies, and hospice services with appropriate vendors and partners.
