Provides clients with transportation and oversight, coordination, and communication of all transports to and from appointments and to aftercare locations.

I. Major Responsibilities:

1. Transport clients to referring sites, interviews, medical services.
2. Transport client and their belongings safely to their residential program placement throughout the state.
3. Conduct daily checks of van to maintain cleanliness of vehicle and inspect exterior for damage
4. Work with Program Director to schedule service for van.
5. Provide transportation for any additional program supplies and emergency needs.
6. Attend and participate in staff meetings, team meetings, and individual supervision.
7. Attend trainings as scheduled
8. Ability to assist outreach case managers with clients to ensure client safety during transport.
9. Demonstrate knowledge base of specific population including the need for maintaining client confidentiality.
10. Effectively communicate with clients, co-workers, and other professionals
11. Performs other related duties

Specialty Responsibilities:

Lead Driver:
1. Check the condition of the van for maintenance and general repairs. Put all repair requests in related to Van maintenance.
2. Provide transportation for any additional program supplies, and emergency needs and/or coordinates/assigns these transports with the other van drivers.
3. Participation in team, staff, and training meetings and support coordination of care, including milieu support.
4. Coordinate and assign all transports weekly with each unit on the continuum, this includes coordination and communication with the management team, other van drivers and destination sites (as needed).
5. Ensure that an accurate schedule is maintained for all transports and all clients arrive to the destinations on time and safely.
6. Oversee, coordinate, communicate and maintain the transportation schedule for all three continuum units - including other van drivers.
7. Communicates with destination sites and facilities around coordinated care, expectations, and transport schedule.
8. Assigns transports to other van drivers throughout the continuum and maintains this schedule.

2. Attends variety of meetings, conferences, and trainings as required or directed.
3. Demonstrates use of quality improvement in daily operations.
4. Complies with all health and safety regulations and requirements.
5. Respects diverse views and approaches and contributes to maintaining an environment of professionalism, tolerance, civility, and acceptance toward all employees, patients, and visitors.
6. Maintains regular, reliable, and predictable attendance.
7. Maintains confidentiality.

II. Position Qualifications:

License/Certification/Education:
Required:
1. High School Diploma/GED/HiSe
2. Current valid US-issued drivers license and a clean driving record

Experience/Skills:
Required:
1. Must be able to pass a CORI background check

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

1. Must be able to remain seated for extended periods of time.
2. Must be able to work in an office environment
3. Must be able to work on a computer 80% of the shift.
