Summary:
Responsible for overseeing and coordinating the activities of the case management team. This position plays a crucial role in ensuring effective patient care coordination, resource utilization, and discharge planning to optimize patient outcomes and enhance healthcare delivery.

Essential Functions:
Provide strong leadership and direction to the case management team, fostering a positive and collaborative work environment.
-Recruit, train, and supervise case managers and support staff.
-Conduct regular performance evaluations and provide feedback for professional development.
-Oversee the coordination of patient care across the healthcare continuum, including inpatient, outpatient, and post-acute settings.
-Collaborate with healthcare providers and interdisciplinary teams to develop and implement patient care plans that address individual needs and preferences.

Education
Bachelor's Degree Social Work required and Master's Degree Social Work preferred

Licenses and Credentials
Physical Therapist [State License] - Generic - HR Only preferred Registered Nurse [RN - State License] - Generic - HR Only preferred

Experience
Experience in case management within a healthcare setting, with some experience in a supervisory or leadership role. 3-5 years required

Knowledge, Skills and Abilities
- Strong knowledge of case management principles, patient care coordination, and utilization management.
- Familiarity with healthcare regulations, privacy laws, and compliance standards related to case management.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work collaboratively with multidisciplinary teams and external stakeholders.
- Proficiency in using healthcare information systems and electronic health records (EHRs).
- Understanding of healthcare reimbursement processes and insurance coverage.
