Full-time, 3-11pm, 11pm-7am

Friends Hospital offers a continuum of care including level of care assessments; specially designed inpatient units for adolescents, adults, and older adults; and a private residential rehabilitation program. The Case Manager: provides quality case management and qualified professional support and services to inpatient social workers as assigned; works with local agency and service providers to facilitate sending and receiving collateral information, treatment referrals, and ongoing communications to support patient's successful inpatient treatment and discharge.

Full-time, permanent night shift position onsite at Friends Hospital.
