Our Family Preservation (FP) program provides home based casework services for multi-problem families. The home-based casework is also available for pre-adoption and post adoption services for adoptive families at risk or in crisis. Benchmark Human Services is hiring an In-Home Case Manager to provide direct service to youth and families by addressing the short-term and long-term needs of the family by working to build upon the strengths of the family. The Case Manager will travel for in-home visits throughout north central Indiana (Cass and Elkhart counties included). Since 1960, Benchmark has been connecting people and potential. General Responsibilities: The In-Home Case Manager will provide direct service to youth and families by addressing the short-term and long-term needs of the family unit - including pre and post adoption. The Case Manager must be willing to travel and have a flexible schedule. Services will be provided in a wide variety of settings, ranging from in-home to various community settings including but not limited to schools, doctors' offices, professional office settings, etc. This position will provide whole-person and family centered care coordination across all domains of human services, especially medical, behavioral, legal and social services.
