Position Summary
This position will serve as a care manager for the Tailored Care Management ( TCM) Program with the Center for Excellence in Community Mental Health ( CECMH). The Tailored Care Management model is a critical element of the NCDHHS Medicaid transformation and BH I/DD Tailored Plans, which will offer robust and effective service models to address the comprehensive health needs of eligible beneficiaries through collaboration with provider-level care management entities. The UNC Center for Excellence in Community Mental Health ( CECMH) has achieved certification as a Care Management Agency ( CMA) to provide care management services to an assigned patient panel of adults with serious mental illness and other behavioral health needs.

This position will engage with assigned beneficiaries through frequent contact determined by client need and acuity and will be responsible for the completion of the care management comprehensive assessment, as well as the development of individual care plans.

This position will complete required trainings and learning activities through UNC CECMH with topics including HIPAA/confidentiality, proficient use of the electronic medical record, corporate compliance and ethics, cultural competence, client rights, workplace safety, as well as areas relevant to the population served. In collaboration with the TCM Program Supervisor, this position will maintain up to date information on the Tailored Care Management model, policies and procedures and expectations as disseminated through NCDHHS and the tailored plan(s).
Minimum Education and Experience Requirements
Bachelor's degree in a Human Services discipline with coursework related to the area of assignment. Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.
Management Preferences

  • Experience working with individuals with severe and persistent mental illness and/ or substance use disorders.
  • Experience providing case management or care management services.
  • Familiarity with 1915i services.
  • Professional interest in integrated behavioral and physical health care for care management.

Required Licenses/Certifications
Candidate must possess a valid driver's license; driving a State vehicle may be necessary.
