Description MaineHealth Corporate Professional - Clinical Summary: Within MaineHealth's Ambulatory Pharmacy Department, the clinical pharmacist will be a part of our growing primary care pharmacy team. In this role, the pharmacist will help provide seamless medication access and medication management for patients. This clinical pharmacist position will be focused on providing high quality, direct patient care through collaborative practice and supporting our primary care teams. In addition, the clinical pharmacist will support the transitions of care for patients. The clinical pharmacist will collaborate closely with other primary care pharmacists throughout MaineHealth. Travel will be required to regional primary care practices located in ourStephens Community and Memorial Community.These primary care practices are located in Norway, ME and North Conway, NH. There may be opportunities for telehealth to make pharmacist daily travel more manageable. Ambulatory care experience with direct patient care strongly preferred. Strong interest in drug therapy management for patients with chronic diseases required. PGY1 Pharmacy residency strongly preferred. Some core functions include:
