Summary:
The role collaborates to enhance the delivery of patient care services along the continuum of care. The resource specialist II meets patients' needs efficiently and expeditiously by continuously improving the patient experience, helping ensure institutional standards of high-quality patient care, reducing costs, and ensuring reimbursement.

Essential Functions
-Research resources and programs available for patients and their caregivers.
-Providing assistance for disabled and elderly assistance programs.
-Coordinate legal, financial, and insurance resources.
-Durable medical equipment contractors and resources for general home safety equipment.
-Supports systems for patients and caregivers experiencing grief or bereavement.
-Provide health education materials, including a hard-copy and electronic storage system that is easily accessible to program staff.
-Assist the Care Manager and Social Worker with follow-up patient support calls.

Education
Bachelor's Degree Business required or Bachelor's Degree Related Field of Study required

Licenses and Credentials

Experience
Five years of which two must be related to healthcare 3-5 years required

Knowledge, Skills and Abilities
- Assessment and problem-solving skills.
- Strong interpersonal skills.
- Strong Customer service skills.
- Ability to work independently with minimal supervision.
- Goal-oriented and accountable in a complex medical environment.
- Experience navigating Insurance Networks is desired.
- Demonstrated organizational and time management skills.
- Strong oral and written communication skills.
- Good computer experience.
