We are seeking a Behavioral Health Navigator to join our community-based team in Colmar, PA, supporting individuals throughout Montgomery County.

The Behavioral Health Navigator plays a key role in coordinating whole-person care by providing expertise in behavioral health assessment, treatment support, and care management for individuals experiencing mental health, substance use, and co-occurring disorders. This position partners closely with healthcare providers, community resources, and consumers to promote integrated care, improve health outcomes, and support individuals in achieving their wellness and recovery goals.

The Behavioral Health Navigator's basic duties include:

Assessment & Care Planning


  • Conduct initial and ongoing assessments of consumers' behavioral health, physical health, and substance use needs, while monitoring response to treatment.
  • Complete all required documentation in accordance with program guidelines and payer requirements, including assessments, releases of information, consents, progress notes, and wellness plans.


Care Coordination & Collaboration

  • Collaborate and communicate with healthcare providers and interdisciplinary team members involved in the consumer's care.
  • Coordinate, complete, and monitor referrals to ensure recommended services are received.
  • Partner with community-based and inpatient physical and behavioral health providers to facilitate communication regarding consumer status during admissions and discharges.
  • Coordinate services with County Mental Health (MH), Intellectual and Developmental Disabilities (IDD), Drug & Alcohol (D&A), Office of Aging, and other relevant community resources.


Consumer Support & Advocacy

  • Educate and empower consumers to utilize self-management strategies that promote active participation in their healthcare and wellness.
  • Assist consumers in obtaining and maintaining essential living needs and life skills, including housing, food security, medical care, recreation, education, and employment.
  • Identify additional support needs and provide appropriate recommendations, advocacy, and referrals to community resources.

Earn$28.43 per hour. (37.5 hours per week)

The ideal candidate will possess the following qualifications:



  • Master's degree in social work, Psychology, Nursing, Counseling, or related field of study.
  • 1 year minimum of direct mental health care experience required.
  • Experience and training in Motivational Interviewing, co-occurring disorders, trauma informed care, and integrated physical health/behavioral health care,preferred.
  • Valid Driver's licenseand clean driving record.
  • Must have a reliable vehicle to use for client visits.
  • Able to work Monday-Friday 8:30am-4:30pm
  • This role is community-based so it requirestravel to various schools, homes, and other community locations to provide services and support clients.Mileage reimbursement is provided.
