Summary: UNC Hospitals Department of Pharmacy has an immediate opening for a highly motivated pharmacist interested in joining the newly established Advanced Clinical Therapeutics Program. The successful Clinical Pharmacist Practitioner (CPP) will play an integral role in the care of patients receiving advanced and complex specialty therapeutics across UNC Hospitals. This innovative pharmacist-led program will provide comprehensive medication management and care coordination services for patients receiving high-cost, high-complexity therapies, initially supporting patients prescribed biologic and specialty medications. The program is designed to optimize therapeutic outcomes, support specialty medication monitoring, and reduce provider burden through a centralized ambulatory care model. The CPP will work alongside a Pharmacy Access and Solutions Technician to navigate specialty therapeutics requiring medication access coordination, financial navigation, safety screening, onboarding support, clinical monitoring, and adherence support. The Advanced Clinical Therapeutics CPP will serve as an Advanced Practice Provider (APP) on the interdisciplinary health care team and will provide clinical support to physicians, advanced practice providers, nurses, specialty pharmacy teams, infusion services, and ancillary staff. Within this innovative care model, the CPP will support initiation and optimization of advanced therapeutics, provide medication safety and efficacy monitoring, coordinate transitions of care, facilitate medication access, and provide longitudinal follow-up for patients receiving specialty medications and biologics. The successful candidate will have an opportunity to provide educational mentorship and experiential precepting to pharmacy students, Post-Graduate Year 1 (PGY1), and Post-Graduate Year 2 (PGY2) Pharmacy residents at UNC Hospitals and work closely with physician trainees and other learners. Additionally, the CPP will work with the Department of Pharmacy and clinic leadership to support institutional and departmental quality improvement initiatives focused on specialty medication access, care coordination, patient outcomes, and operational efficiency. Academic appointment at the UNC Eshelman School of Pharmacy is available commensurate with experience. Completion of a PGY2 specialty residency in Ambulatory Care, Pharmacotherapy, Specialty Pharmacy, or another related specialty residency or fellowship, or an equivalent level of clinical experience, is preferred. Three to five years of ambulatory care, specialty pharmacy, or advanced therapeutic management experience is strongly preferred. Board certification in Ambulatory Care, Pharmacotherapy, or another relevant specialty, and prior experience as a resident and student preceptor, are preferred.
