Overview: Program Manager, Community Transitions

Sign on Bonus Available with this position - $5,000

This full-time position provides leadership as the Program Manager of the Comprehensive Class Member Transition Program. This program works with our multi-disciplinary team focused on class members served in the Williams Consent Decree. Williams Consent Decree works towards moving individuals with severe and persistent mental illness and dual disorders wanting to transition from living in a SMHRF (Specialized Mental Health Rehabilitation Facilities) to community settings. This position supports the implementation of the Williams Consent Decree programs in achieving outcomes and targets. This leader will provide clinical and administrative oversight to teams providing outreach, assessment/evaluations, transition coordination, quality and data monitoring, and clinical care management needs of all eligible class members transitioning from long-term care.

Core Responsibilities Include:

  • Oversee overall operations for the data management, and clinical care management teams in the provision of a full array of services to Williams and Colbert members being transitioned out of SMHRF and those still in facilities with service plans to work towards transition;
  • Ensure that care provided integrates all aspects of members lives including mental/emotional health, physical health/wellness, employment and education, social and family relationships, substance use/abuse issues, spiritual, community-integration, and housing needs;
  • Submit timely and accurate reports and invoices for reimbursement of Comprehensive Class Member Transition Program;
  • Oversee completion of daily data management and weekly, monthly, and quarterly reports with timely submission of reports;
  • Coordinate with Program Managers of Assertive Community Treatment, Prairie Street Group Home, West Main and Orchard Street Group Homes, and the Director of Nursing to ensure all team members are working together effectively in care provision and to ensure transition of Williams Class Members into aftercare services;
  • Monitor program staffing needs and participate in hiring and orientation of new staff;
  • Coordinate communication, service provision, and continue to build relationships with area providers for linkage outside of our service area;
  • Ensure that all clinical activities performed by the team are compliant with standards, policies and procedures of the organization.

Education and Experience:

  • Master's Degree in counseling, social work, psychology or human service related field is required;
  • Licensure as an LCSW, LSW, LCPC, or LPC is required within two years of employment.
  • A valid driver's license, reliable means of transportation, and proof of current automobile insurance is required for this position.

Salary Range: $65,000-$85,000 per year (dependent on education, experience and licensure)
