SUMMARY:

Responsible for providing social work services to patients and families. Performs in accordance with the facility's policies and procedures. Follows the facility's standards for ethical business conduct. Conducts self as a positive role model and team member. Participates in facility committees, meetings, in-services, and activities.

KEY RESPONSIBILITIES:

* Assesses patient and family psychosocial needs and develops plan of care in concert with physicians, nurses and therapists.

* Links patient/family to appropriate community resources including information/referral to sources of financial assistance, transportation, food, housing, support groups.

* Provides short-term counseling to identify problems and facilitate needed referrals.

* Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care.

* Documents patient/family status and history, diagnosis, treatment plan, evaluation results, observations and progress in medical record.

* Serves as a resource to other staff on community resources and in area of specialty.

* The job holder must demonstrate current competencies for job.

REQUIRED QUALIFICATIONS:

* Strong communication skills with patience

* Ability to work under pressure and manage emotional situations

* Strong ethics and confidentiality EDUCATION:

* Degree in social work from an accredited program, which can be a Bachelor of Social Work (BSW) or a Master of Social Work (MSW).

* Proof of Licensure, LSW, LMSW, LCSW (depending on level of education and state)

* Additional course work in area of specialty (e.g., psychiatric, pediatric) as required.

EXPERIENCE:

* Two years' experience in social work with broad background with diverse patient populations, preferably in medical setting. Additional one year experience in area of specialty.

* Ability to interact effectively as member of interdisciplinary health care team.

* Ability to use community resource database computer software effectively.

* Ability to communicate appropriate with diverse patient populations and conduct effective community outreach activities.

KNOWLEDGE:

* Legal and Ethical Knowledge

* Child and Family Services/Healthcare Social Work

* Communication and Interpersonal Knowledge

* Knowledge of how to use variety of diagnostic tools and techniques including DSM IV-R.

* Knowledge of the development and maintenance of information/referral systems.

While performing the duties of this job the employee is frequently required to walk; stand; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; and talk or hear. The employee is occasionally required to sit or stand. May be exposed to various patient conditions including infectious diseases, blood, and bodily fluids.
