Position Overview: The Trauma-Focused Community-Based Services Care Coordinator Supervisor provides trauma-informed services to children and youth, with a special focus on individuals at risk of or involved in Commercial Sexual Exploitation of Children (CSEC) and other forms of sexual abuse. In addition to supervisory responsibilities, this position requires direct care involvement as needed, ensuring that services are delivered effectively and compassionately. The Supervisor will provide guidance, training, and oversight to the team while also collaborating with clinicians, families, schools, and community partners to support youth recovery and resilience.

Essential Functions:

  • Lead, supervise, and support a team of Trauma-Focused Community-Based Care Coordinators, ensuring quality service delivery and adherence to trauma-informed practices.
  • Provide regular supervision, coaching, and professional development opportunities for team members.
  • Oversee case management, workload distribution, and service delivery to ensure compliance with organizational standards.
  • Provide trauma-informed and culturally sensitive direct care to children, youth, and families as needed, in collaboration with the clinician.
  • Participate in the 24/7/365 on-call rotation to support crisis intervention and care.
  • Assist with safety planning, risk assessment, and stabilization for youth during critical situations.
  • Work closely with clinicians, peer mentors, medical professionals, educators, and other partners to ensure comprehensive care.
  • Coordinate multidisciplinary team meetings, case reviews, and collaboration sessions with community stakeholders.
  • Support the development and implementation of evidence-based and trauma-informed therapeutic interventions.
  • Monitor program outcomes and participate in continuous improvement initiatives.
  • Assist in ensuring that individualized recovery goals and aftercare planning are effectively implemented for youth.
  • Ensure timely and accurate documentation by staff in the Agency's Electronic Health Record system, EchoVantage, ensuring compliance with regulatory and organizational standards.
  • Oversee the completion of required assessments and reports, including pre and post-tests for program evaluations.

Requirements

  • Bachelor's degree in Social Work, Psychology, or a related field. Required
  • Valid driver's license and reliable transportation. Required
  • Bilingual (English/Spanish)-Preferred
  • A minimum of three years of experience providing trauma-informed care.

Knowledge, Skills, and Abilities:

  • Experience working with at-risk youth and families preferred.
  • Experience with foster care/families formed through adoption.
  • Strong understanding of family dynamics, youth development, and evidence-based therapeutic approaches.
  • Ability to work independently and as part of a multidisciplinary team.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work collaboratively within a multidisciplinary team and with external partners.
  • Flexible and adaptable to providing culturally competent and client-centered care.

Salary Range: $50,000-$54,500
